Kayaba Shock Rebuild need p/n's

I need specs to rebuild the rear shock.

From what I've gathered so far:

5W oil
150 psi nitrogen charge
44mm x 16mm seal head

Can anybody confirm?
Does anybody have a source and/or part number for the seal head? I'm finding multiple part numbers for 44/16 heads.
